Residents 5 and 8 were cognitively impaired and had no documented advance directive or POLST, and there was no evidence that they or their responsible parties were offered or assisted in formulating one. Residents 9 and 10 had no cognitive impairment, but their active physician orders also lacked any advance directive or POLST, and there was no documented evidence that they were offered or assisted in formulating an advance directive. The Administrator confirmed there were no advance directives or POLSTs in the records and that Resident 2's physician order should have reflected the resident's wishes.
