Summary

The facility failed to implement sufficient safety interventions and supervision to prevent elopement for a resident with a history of wandering and unauthorized exits. The resident’s record showed diagnoses including bipolar disorder, unspecified psychosis, impulsiveness, suicidal and homicidal ideations, and auditory and visual hallucinations. Elopement evaluations repeatedly identified the resident as at risk, with scores ranging from 7.0 to 10.0, and the resident had a recent re-admission from a hospital stay related to mental health concerns. The resident repeatedly left or attempted to leave the building and repeatedly removed secure care devices. Facility documentation described multiple incidents in which the resident exited the facility, was found on or near roads away from the building, and stated he wanted to see how far he could go. On several occasions, the resident cut off the secure care bracelet using utensils obtained from the dining room, including a dinner knife, butter knife, or knife from the silverware area. The resident was also documented wandering the halls, entering other residents’ rooms, and attempting to go out the front door. The record also showed that the resident obtained sharp objects from accessible areas within the facility. Staff documented that knives were present in dining areas, that the resident retrieved utensils from the dining room and dirty dish bin, and that scissors were accessible at the nurse’s station. A one-on-one sitter was used as an intervention, but one note stated the sitter left the resident briefly to assist another resident and returned to find the resident gone. Survey observations later found butter knives on dining tables, knives in the dirty dish cart, and scissors, a screwdriver, pliers, and fingernail clippers accessible at nurse stations.
