Failure to Notify Providers When Ordered Antibiotics Were Not Given

Summary

The facility failed to notify the medical provider when ordered antibiotic medications could not be provided for four residents. Facility policy required next-day notification to physicians/APPs and licensed nurses, and staff stated notifications were to be made through Care Communications or, if by telephone, documented in a progress note. However, the records reviewed showed that provider notification was not documented for any of the four residents when their ordered antibiotics were not administered. Resident R1 had an order for IV ceftaroline fosamil for sepsis, but the June 2026 MAR showed the medication was not given from admission through 6/24/26, when the resident was hospitalized to receive the missed antibiotic. Progress notes did not show notification to the NP or physician, and the only Care Communications entry was written on 6/22/26 after pharmacy reported a copay issue and asked whether the resident had been receiving the IV antibiotic. The attending physician later stated he was notified through the communication board six days after the resident arrived and said he should have been contacted immediately. Resident R2 had an order for rifaximin for hepatic encephalopathy and missed 14 of 60 scheduled doses in June 2026, with no progress note or Care Communications entry showing notification to the NP or physician. Resident R3 had orders for ceftriaxone and linezolid IV, and the MAR showed multiple missed doses of both medications in June 2026; again, no notification to the NP or physician was found in the records. Resident R4 had an order for meropenem IV three times daily and missed several scheduled doses beginning with the first day of therapy, with no documentation that the NP or physician was notified. During interview, the attending physician confirmed he had not been made aware that Residents R2 and R3 had not received their ordered antibiotics, and the NHA and Interim DON confirmed the failure to notify the medical provider for all four residents.
