Resident #25 was admitted with diagnoses including COPD, schizophrenia, major depressive disorder, anxiety disorder, nuclear cataract, bilateral primary open-angle glaucoma, and convulsions. The resident had a BIMS score of 15, indicating intact cognition. The record showed no baseline care plan, and the comprehensive care plan was started 7 days after admission. Resident #28 was admitted with diagnoses including type 2 diabetes mellitus with diabetic neuropathy, parkinsonism, multiple sclerosis, paroxysmal atrial fibrillation, acute diastolic CHF, COPD, and end stage renal disease. This resident also had a BIMS score of 15, no baseline care plan was found, and the comprehensive care plan was started 6 days after admission.
