Failure to Prevent Falls, Unsafe Transfers, and Inadequate Post-Fall Assessment
Summary
The facility failed to ensure residents at risk for falls were protected by the ordered fall interventions and by adequate supervision during transfers and after falls. For Resident #113, who had severe cognitive impairment, dementia, spinal stenosis, hospice services, and orders for a perimeter mattress, bed bolsters, bilateral half side rails, floor mats, and two staff for transfers, video surveillance showed the resident on the floor mat beside the bed and then being lifted by one LPN and thrown back into bed without an assessment for injury. The video also showed that the ordered perimeter mattress, bed bolsters, and bilateral half side rails were not in place, and the nurse left the resident alone afterward. The resident later had a swollen right arm and hand and was hospitalized with a thoracic burst fracture and multiple bruises to the hands, wrists, forearms, and upper extremities. The facility also failed to timely identify and investigate unexplained injuries for Resident #56, who had dementia, severe impairment in decision-making, and was dependent on staff for ADLs and transfers with a mechanical lift and two staff assistance. Bruising under the right breast and mid-chest was first documented, then spread to the right upper extremity, shoulder, and breast with pain and edema. The resident was later sent to the hospital and diagnosed with multiple bilateral rib fractures and a large hematoma, with hospital paperwork noting bruising had started earlier than the facility documented. The record showed no progress notes for a period before the injury, and the facility did not initiate the investigation until after the hospital admission. The facility also failed to complete adequate fall assessments and investigations for Resident #115 and Resident #6. Resident #115, who had vascular dementia and a history of falls, was found with a bruise under the right eye after an unwitnessed fall, but the record did not contain an adequate investigation of what caused the fall, what interventions were in place, or what new interventions were implemented afterward, and staff statements were not obtained. Resident #6, who had moderate to severe cognitive impairment and dependence for rolling and ADLs, was found face down on the floor between the bed and window after a fall, and the record showed no fall risk assessment before the event. The report also states the facility failed to implement adequate interventions to prevent a fall, prevent accident hazards, conduct a safe resident transfer, provide appropriate assessment and care after a fall, and complete timely and accurate fall assessments, investigations, and care plans.
