Hand Hygiene During Tracheostomy Care and TB Screening Compliance

Good Shepherd VillageSpringfield, Ohio Survey Completed on 06-30-2026

Summary

The facility failed to ensure proper hand hygiene during tracheostomy care for Resident #5. Resident #5 was admitted with diagnoses including non-traumatic intracranial hemorrhage and tracheostomy status, and the quarterly MDS showed the resident was unable to complete the BIMS assessment. Physician orders directed daily tracheostomy and neck care, daily drain sponge changes, daily tracheostomy mask changes, weekly tracheostomy collar tubing changes, and Enhanced Barrier Precautions related to the tracheostomy and gastrostomy tube. During observation, the RT entered the room wearing a gown and gloves and, while wearing the same gloves, touched storage boxes, supply bins, and unopened supply packages in the resident’s closet before opening the sterile tracheostomy care kit and handling clean supplies. The RT continued the procedure without removing the contaminated gloves or performing hand hygiene after contact with environmental surfaces and before handling clean supplies or the resident’s tracheostomy. The RT removed the soiled drain sponge and inner cannula while still wearing the same gloves, then placed a second pair of gloves over the original contaminated gloves to insert the new inner cannula. The RT later removed only the outer pair of gloves, leaving the original gloves in place, and used those gloves to wipe the tracheostomy site and handle a clean drain sponge. During interview, the RT confirmed she did not perform hand hygiene or replace the original gloves during the procedure after contacting environmental surfaces. The facility also failed to ensure TB Mantoux screenings were completed according to its TB Risk Assessment. Review of personnel files showed CNA #112, CNA #145, and LPN #146 provided resident care before their baseline TB Mantoux tests were read. The personnel file for LPN #108 and CNA #192 contained annual TB screening questionnaires, but the Human Resource Director confirmed they were not completed according to the facility’s TB Risk Assessment, which required baseline TB screening upon hire and annual TB symptom screening thereafter.

D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A resident being evaluated for C. difficile was not consistently managed under the correct contact-enteric precautions. Failure to Use EBP PPE During Wound Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Failure to Use EBP PPE During Wound Care: A resident with a wound and severely impaired cognition was receiving ordered wound care when the DON and ADON entered the room, washed hands, and donned gloves but did not wear gowns before starting care. EBP signage and PPE were present at the bedside, and both leaders later stated they forgot to put on gowns even though the resident was on EBP for an open wound and the facility policy required gown and glove use for wound care.

Failure to Follow Enhanced Barrier Precautions and Hand Hygiene During Resident Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Staff failed to consistently follow EBP and hand hygiene for two residents who required high-contact care. One resident with an indwelling catheter and cognitive impairment did not have EBP followed during transfers, clothing removal, and pericare, and another resident with a suprapubic catheter and multiple pressure ulcers had staff wear PPE incorrectly, remove PPE in the room without hand hygiene, and continue care after emptying the catheter bag without changing gloves or cleaning hands. Interviews confirmed staff were expected to use gowns, gloves, and hand hygiene for these tasks.

Short Summary

A resident on Contact Precautions for active C. difficile infection was not consistently protected by required PPE. Although signage outside the room directed all entrants to wear a gown and gloves, an RN entered the room with medications without either item and touched the bedside table and door surface. The resident stated staff did not always wear gowns, and the DON confirmed staff were expected to wear a gown and gloves when entering the room while precautions were in place.

E
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A facility failed to maintain infection control during care for two residents. During incontinence care, a CNA removed gloves and applied clean gloves without hand hygiene before placing a clean brief on a resident who was dependent for toileting hygiene and always incontinent. During ostomy care, an LVN handled a resident’s colostomy, wiped stool from the stoma, and continued care without removing soiled gloves, sanitizing hands, or putting on clean gloves. The DON stated staff were expected to perform hand hygiene between glove changes.

Infection Control Failures During Incontinent Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Infection Control Failures During Incontinent Care: Two residents were observed receiving incontinent care with multiple infection control lapses. A CNA did not perform hand hygiene between glove changes or after glove removal while caring for one resident, and another CNA did not wash hands before care, changed nothing between dirty and clean tasks, touched clean items with dirty gloves, placed soiled linens on the floor, and handled dirty linens with bare hands after glove removal. The DON stated hand hygiene, glove changes, and proper handling of soiled linens were required, and facility policies reflected those practices.
