Meal Service Failed to Preserve Resident Dignity

Divine Rehabilitation And Nursing At Shane HillRockford, Ohio Survey Completed on 06-11-2026

Summary

The facility failed to serve meals in a manner that preserved resident dignity during lunch service. During observation, 14 residents were seated in the dining room at seven tables, and meals were served sporadically rather than to all residents seated together at the same table. At one table, two residents were already eating for five minutes before the last resident at that table, Resident #45, was served. Resident #45 was cognitively intact, had diagnoses including depression and anxiety, and stated that she did not enjoy sitting at the table and waiting for her food while the other two residents ate, noting that this happened regularly. A CNA confirmed that the residents were not served in a dignified manner and stated it would be better if all residents at the table were served simultaneously. The meal service for the Memory Care Unit was also delayed beyond the posted lunch time. The posted meal time for the 100 hall was 12:00 P.M., but family and staff reported that trays were routinely delivered after 12:15 P.M. and sometimes closer to 12:30 P.M. Resident #10, who had diagnoses including Alzheimer's disease, dementia with agitation, macular degeneration, heart disease, and thrombocytopenia, no longer ate in the dining room according to the family representative because of the cleanliness and condition of the dining room, and instead ate in her room, which delayed tray delivery. Observation showed Resident #10's tray was brought to her room after the posted lunch time. An LPN verified that meal service was always after the posted time and that there had been resident and family complaints about the late meal service. During observation of lunch service, the Memory Care Unit trays were not transported out of the kitchen until 12:15 P.M., and only one CNA was delivering trays for the unit, delaying meals for all 20 residents on that unit.

Short Summary

A resident with dementia and Alzheimer's disease had his beard completely shaved by two CNAs without his RP's consent. The RP stated the resident had worn his beard for many years and had not been seen without it, and the DON stated the RP should have been notified because the resident lacked capacity to make decisions for himself. The facility policy required staff to ask the resident or, when appropriate, the resident representative about grooming preferences upon admission.
