Food Served at Unappetizing Temperatures and Quality

Laurels Of Massillon, TheMassillon, Ohio Survey Completed on 05-14-2026

Summary

The facility failed to ensure residents received appetizing food served at palatable temperatures. Seven residents interviewed reported concerns with the quality of meals, including food described as bland, tasteless, undercooked, mushy, unappealing, and served cold. One resident stated the food was especially undercooked on weekends, another said he had to order take-out because of the taste, and another reported not receiving fresh fruit except bananas and that hot foods were sometimes served cold. Other residents reported bland food, overcooked vegetables, and hot foods served cold. During observations of the lunch tray line and test tray service, food temperatures were measured and verified by Dietary staff. Chicken patties, roasted potatoes, and green beans were observed at temperatures of 189 degrees F, 169 degrees F, and 181 degrees F on the tray line. A new pan of roasted potatoes was later observed coming out of the oven at 167 degrees F and placed for lunch tray line service. When a test tray was taken from the kitchen to the dining room on an open cart and then checked, the chicken patty measured 107 degrees F and was lukewarm, the roasted potatoes measured 92 degrees F and were lukewarm, and the green beans measured 123 degrees F and were warm. The Dietary Manager verified these findings and stated hot foods should ideally be served between 120 and 130 degrees F.

D
F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

Two residents were served microwave-reheated items without a temperature check before service. A staff member reheated almond milk and chicken noodle soup, placed both on the counter, and they were later measured at 182 F and 190.4 F, respectively. Staff stated the kitchen checked temperatures, but the items were served before confirming they had cooled to a safe temperature.

F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

A resident with cognitive impairment and a mechanically altered diet was left sitting at the dining table with an uncovered lunch tray for nearly 45 minutes without staff cueing or feeding assistance, despite needing supervision and help with meals. When an NA finally began feeding her, the meal items were found to be lukewarm or cold, and neither the NA nor the dietary manager reheated the food or replaced the tray.

Food Portions Not Measured Per Recipe
F
F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

Food portions were not served per recipe directions when a cook prepared sandwiches, pureed items, cake, and side items by estimation instead of measured amounts. The cook used tongs to add unmeasured meat, onions, and cheese to buns, served visibly different cake portions, and confirmed there were not enough pickles for all residents. The cook stated the portions were “eye balled” and that the spoons were inconsistent.
