Incomplete PASARR Assessments and Missing PASARR Care Plans
Summary
The facility failed to ensure Preadmission Screening and Resident Review (PASARR) assessments were complete and failed to ensure PASARR recommendations were implemented for two residents reviewed. One resident was admitted with multiple psychiatric and neurologic diagnoses, including mood disorder, dementia, schizoaffective disorder, PTSD, visual hallucinations, delusions, mild cognitive impairment, anxiety, major depressive disorder, hemiplegia, hypertension, transient ischemic attack, cerebral infarction, and osteoarthritis. The resident’s orders included lorazepam 0.5 mg by mouth twice daily for delusional disorders, and the care plan noted psychotropic medication use related to dementia, anxiety, delusional disorder, and schizoaffective disorder. However, the resident’s PASARR did not document the diagnosis of delusional disorder, and the Social Service Designee confirmed this omission during interview. A second resident’s PASARR did not reflect delusions, anxiety, or paranoid personality disorder, even though the resident was admitted with schizoaffective disorder bipolar type, delusional disorders, bipolar disorder, major depression, anxiety, and paranoid personality disorder. The annual MDS also stated the resident was not considered by the State Level II PASARR process to have a serious mental illness and/or intellectual disability or related concern, despite triggering for Level II evaluation due to schizoaffective disorder, bipolar disorder, dementia with behavioral disturbance, and delusional disorder. The Level II recommendations included ongoing psychiatric medication review, neurological exams, mental health counseling, a safety plan, a behavioral-based treatment plan, therapy, activities, and medication as ordered, but the current plan of care contained no PASARR plan of care or safety plan. The SSD and MDS Nurse both confirmed the PASARR and MDS were inaccurate and that no PASARR plan of care or safety plan was in place.
