Resident #81 was admitted with diagnoses including heart failure, chronic kidney disease stage four, anxiety disorder, and an unspecified fracture of the fourth thoracic vertebra. Hospice medication orders dated 07/09/26 included Oxycodone 5 mg by mouth every four hours for pain and Oxycodone 5 mg by mouth every four hours as needed for pain, but the facility admission orders dated 07/10/26 only included Oxycodone 5 mg by mouth every four hours as needed for pain and did not transcribe the scheduled every-four-hour order. Medication administration records and narcotic sign-off sheets showed the resident received Oxycodone 5 mg on 07/10/26 at 3:31 P.M. and not again until 07/11/26 at 7:45 A.M. Pain assessments documented pain levels of 9 and 10 on 07/10/26, with no documented pain assessment after 6:09 P.M. that day until 5:59 P.M. the next day. Nursing notes stated the resident was crying and reported back pain of 10 at 7:45 A.M. on 07/11/26, and the hospice RN later confirmed the intended orders were Oxycodone 5 mg every four hours and Oxycodone 5 mg as needed for breakthrough pain every four hours. The clinical manager verified the medication was transcribed incorrectly, resulting in missed doses during the night.
