Delayed Reordering of Antianxiety Medication

Summary

The facility failed to ensure timely reordering of a resident’s antianxiety medication, Alprazolam (Xanax) 0.25 mg by mouth three times daily for panic disorder. The resident was admitted with diagnoses including panic disorder, psychoactive substance use, major depressive disorder, chronic pain, and insomnia, and had a BIMS score of 15 indicating intact cognition. Her care plan addressed altered mood related to depression, panic disorder, and anxiety and directed staff to administer medications as ordered and monitor for side effects and effectiveness. Progress notes and the MAR showed repeated gaps in the resident’s Xanax administration because the medication was pending from the pharmacy, not available, awaiting a new script, and later awaiting arrival from the pharmacy. The resident missed nine scheduled doses in July 2026. During observation, she was tearful and reported she had gone two days without her scheduled Xanax and was experiencing a lot of anxiety, stating the facility did not stay on top of ordering her medication and that this was a frequent occurrence. Staff interviews confirmed the prescription had run out of refills, the facility did not attempt to obtain a new prescription until after the medication was already unavailable, and although the medication was available in the contingency box, it was not pulled for administration.
