Incorrect Daily Nurse Staffing Census Postings

The Rosewood Health CenterRaleigh, North Carolina Survey Completed on 07-16-2026

Summary

The facility failed to ensure the daily nurse staffing posting had the correct resident census on 15 of 23 days reviewed across two date ranges. The Daily Nurse Staffing Information showed census counts that did not match the facility’s detailed census report on multiple dates, including instances where the posted census was higher or lower than the census reflected in the detailed report. The discrepancies were identified during record review of the staffing postings and census reports. During interviews, the DON stated the Social Worker sent the resident census created at midnight to a member of the leadership team, and that bed holds could sometimes cause a discrepancy in the census on the Daily Nurse Staffing Information. The overnight nurse was responsible for posting the Daily Nurse Staffing Information, and the Medical Records Coordinator was responsible for making changes if needed. The Social Worker was unsure whether the report she sent was included in the census on the staffing information. The Medical Records Coordinator stated the night shift nurse was responsible for posting the information and later acknowledged she had not been updating the daily resident census before the date of the interview. The Administrator stated he was not sure of the process for updating the resident census when there were admissions or discharges. Night shift Nurse #4 stated she completed and posted the staffing information according to the midnight census by counting residents in each hallway.

Short Summary

Failure to Post Required Daily Staffing Information: The facility did not post the required daily census and actual DHPPD information for the prior day, even though only the next day’s estimated staffing form was displayed in the posting area. The ADM said the Scheduler posted the forms before leaving, but the Scheduler stated she had only been instructed to post the next day’s estimated form, not the prior day’s actual census and DHPPD data. The DON confirmed the required prior-day form was not posted, despite the facility policy requiring daily staffing postings in a prominent location accessible to residents and visitors.

Inaccurate daily nurse staffing posting
D
F0732 F732: Post nurse staffing information every day.
Short Summary

Inaccurate daily nurse staffing information was posted in the window at the main entrance and did not reflect the actual resident census or the hours worked by RNs, LPNs, CMAs, and CNAs. Staff said receptionists completed the posting each morning, used the schedule inconsistently, did not know which nurses were RNs or LPNs, and did not update the posting as staffing changed. The facility had no policy for the posting, and the Facility Assessment called for staffing to be adjusted based on census and resident care needs.
