Failure to Apply Ordered Hand Splint

Summary

The facility failed to apply a right-hand C-grip splint as ordered for a resident with stiffness of the right hand, hemiplegia, hemiparesis, muscle weakness, and dementia. The resident was admitted with diagnoses including cerebrovascular disease affecting the dominant right side and was care planned to wear the splint during nighttime hours for 8 hours or as tolerated. Occupational Therapy discharged the resident after he was able to tolerate the splint for 8 hours and documented that the splint was intended to prevent further contractures of the hands, wrists, and fingers. The physician order dated 5/27/26 directed the resident to wear the RIGHT C-grip SPLINT during nighttime hours for 8 hours or as tolerated. The Occupational Therapy discharge instructions stated nursing staff had been trained on splint application, and the resident’s care plan continued to include nighttime splint use. However, review of the MAR, TAR, and POC documentation for June and July 2026 showed no documentation that staff applied or removed the splint. During observation, the resident was seen in a wheelchair without the splint. A nurse assigned to the resident stated she was not aware the resident had a splint, had not applied splints, and had not observed the resident wearing one at night. The Rehabilitation Director stated the resident had been under OT because the splint was misplaced and that therapy had recommended nighttime use. The Unit Manager stated the splint order had not been transcribed into the MAR or TAR, so staff could not see the order or document application and removal, and she did not believe staff were applying the splints at night. The splint was later found stored in the bottom drawer of the nightstand.
