Summary

The facility failed to notify the physician of missed tirzepatide administrations for one resident with diabetes and sleep apnea. The resident was admitted with diagnoses including diabetes and sleep apnea, had a hemoglobin A1C of 6.8 on 9/11/25 and 7.1 on 5/29/26, and weighed 362 pounds on 9/5/25 and 384 pounds on 6/9/26. A physician order dated 11/19/25 prescribed tirzepatide 10 mg weekly, later increased to 12.5 mg weekly on 2/27/26, and again documented as weekly on 7/4/26. The MAR showed multiple missed doses of tirzepatide, including on 1/19/26, 3/5/26, 4/2/26, 5/21/26, 6/11/26, and 7/18/26. During interviews, multiple nurses stated they had not called the physician or on-call provider, and the resident reported missing doses for months.
