Summary

The facility failed to reconcile conflicting oxygen orders for a resident with chronic respiratory failure, tracheostomy status, quadriplegia, dysphagia, and recurrent pneumonia after readmission from the hospital. The resident’s record showed an active physician order for humidified oxygen to the tracheostomy at 3 liters every shift, entered as a verbal order from the Medical Director, while the MAR reflected that order as being administered over multiple shifts. However, a Nurse Practitioner note documented a recommendation for tracheostomy oxygen at 6 liters per minute, creating conflicting directions for the resident’s respiratory care. During observation of tracheostomy care, the resident was noted to breathe through the tracheostomy with a trach collar connected to oxygen tubing. The resident’s oxygen saturation was 97% before care and 98% after care, and the oxygen concentrator was observed set at 10 liters. The nurse stated the resident was ordered 10 liters to keep oxygen saturation above 90%, but could not locate a physician order for that amount. The Respiratory Therapist later observed the concentrator set at 10 liters, stated he did not recommend that setting, and turned it down to 6 liters. Interviews showed staff uncertainty about the correct oxygen flow rate. The DON stated staff were familiar with the resident and would have recognized that 3 liters was not enough, and that nursing should have contacted the NP for new orders. The Medical Director stated he was not sure why the resident was on 10 liters and did not recall giving the 3-liter order, while the NP stated she verbally ordered 6 to 10 liters per minute after hospitalization but the order may not have been entered into the electronic record. The Administrator stated staff were expected to administer oxygen per physician orders and that the NP’s verbal order may not have been entered by the nurse.
