Insufficient Nursing Staffing and Gaps in Licensed Nurse Coverage

Summary

The facility failed to maintain sufficient nursing staffing to ensure the highest practicable physical, mental, and psychosocial well-being of each resident, and it failed to have licensed nursing coverage 24 hours per day. The facility’s assessment dated 10/28/26 identified the need for four licensed nurses providing direct care, six nurse aides, and four nursing personnel with administrative duties each day. Record review of staff timecards from 06/24/26 through 07/24/26 showed multiple shifts when staffing did not meet those levels, including shifts with only one nurse, shifts with only two CNAs, and shifts when no nurse was present during part of the day or night. A grievance dated 07/13/26 stated that a resident requested a shower during the night shift over the prior weekend but was told he could not shower because only two CNAs were available. During interviews, the resident stated he had waited over an hour for staff to respond to his call light and that the Administrator assisted with showers because there were not enough staff available. Multiple staff members stated the facility was short-staffed, that showers were delayed or could not be completed, and that charting and other duties were not always finished because staff were needed on the floor. Interviews also described periods when licensed nursing coverage was not maintained. Staff reported that on one day there was no nurse on shift for about three hours, and on another day the DON became ill after working approximately 33 to 36 hours straight and left the facility, resulting in a period without nurse coverage until another nurse arrived. The Ombudsman also reported receiving an anonymous complaint that no nurse was on shift on 07/08/26. The Administrator acknowledged that the facility needed to be staffed according to the facility assessment and stated that resident care had not declined.
