Review of the facility policy titled Vulnerable Adult showed the facility required background checks and verification of certification status for nursing assistants and stated it would not employ individuals with findings of abuse, neglect, exploitation, mistreatment, or misappropriation of resident property. However, review of employee files showed no evidence that Van Driver-U, hired 4/15/26, or Universal Worker-V, hired 4/29/26, had State Nurse Aide registry background checks completed at the time they were hired. During interview, the Business Office Manager confirmed the facility was not checking the Nurse Aide/Medication Aide Registry for negative findings for all newly employed staff.
