Summary

The facility failed to provide a functioning resident call system in multiple rooms, including four rooms on the 500 hall where the call lights did not sound at the nurses’ station. Observation showed the call light illuminated at the room door and on the nurses’ station panel, but no audible sound was noted and no staff responded while the resident remained waiting. Resident #3 had pneumonia and chronic pain and was assessed as having severe cognitive impairment, using a wheelchair, and requiring substantial to maximal assistance with eating, oral hygiene, toileting hygiene, showering, dressing, and personal hygiene. A visitor reported that the resident’s call light was on for about 90 minutes on one evening and no staff responded while the resident yelled for water, so the visitor assisted with water. Resident #4 reported that staff sometimes took 30 to 45 minutes to respond to the call light and did not know whether it made a noise when pressed. Testing by maintenance, the DON, and the ADON confirmed that four out of four rooms on the 500 hall did not have an audible sound when the call lights were tested.
