Care plans did not address assessed needs or medication monitoring
Summary
The facility failed to develop and implement individualized comprehensive care plan interventions for three residents whose assessed needs and physician-ordered treatments were not fully addressed in their care plans. The facility policy stated that comprehensive care plans must include measurable objectives, time frames, and all services identified in the resident’s comprehensive assessment to meet medical, nursing, mental, and psychosocial needs. For one resident with frequent pain and a BIMS score of 11, the care plan included pain-related interventions and administration of prescribed pain medication, but it did not include individualized directions for staff on how to manage pain during activities of daily living, repositioning, or turning. The resident reported pain during turning, repositioning, movement, and dressing and stated pain medication was not routinely offered before bathing or ADL care. A CNA and RN both confirmed the resident experienced pain during repositioning, transfers, bathing, turning, and ADL care, and the MDS/Care Plan nurse confirmed the care plan did not include individualized interventions for those situations. The resident had an order for Hydrocodone-Acetaminophen every 8 hours as needed. For another resident with hemiplegia, dementia, epilepsy, muscle weakness, poor safety awareness, and a BIMS score of 12, the care plan addressed fall risk and abnormal bleeding risk, but the call light was observed wrapped around the overhead bed light fixture and out of the resident’s reach on multiple survey observations. The resident was receiving Duloxetine, Rexulti, Trazodone, and Xarelto, and the record showed no documentation of monitoring for potential adverse consequences of these medications. RN staff confirmed there was no documentation area on the MAR for such monitoring, and the DON confirmed there was no documentation showing monitoring for adverse consequences. For a third resident with Parkinson’s disease, anxiety, depression, and a BIMS score of 12, the care plan addressed altered level of comfort and included monitoring for adverse side effects such as lethargy, slurred speech, sedation, or confusion. The resident was receiving Aripiprazole, Fluoxetine, Hydroxyzine PRN, and Trazodone, but the MAR and medical record contained no documentation that the resident was monitored for medication side effects. RN staff and the DON confirmed there was no documentation demonstrating monitoring for adverse consequences, and the DON stated residents receiving psychotropic, anticoagulant, and anti-anxiety medications were expected to be monitored each shift.
