Unsafe and Unhomelike Hallway Carpeting and Damaged Ceiling Tiles

Lakeshore Rehabilitation Center LlcWaseca, Minnesota Survey Completed on 07-16-2026

Summary

The facility failed to maintain a clean, comfortable, and homelike environment in the East and [NAME] hallways. Observation of the East hallway identified multiple dark stains on the carpeting, worn and soiled carpet, fraying seams, and areas where the carpet was worn through with the backing visible. Staff acknowledged the carpet was stained, frayed, not securely attached at the seams, and had been shampooed and scrubbed multiple times without improvement because the carpeting was old. The environmental services manager stated requests for replacement flooring had been ongoing for an extended period and described the condition as embarrassing and unfair to residents. The maintenance director and administrator confirmed the carpeting was not secure, frayed, and could be tripped on, and stated replacement had been discussed but no replacement date had been established. The administrator acknowledged residents would not be expected to live with frayed carpeting or holes in the carpet because it is not a homelike environment. In the East hallway bath tub room, observation identified a ceiling tile with a large brown discolored area around the sprinkler head, an adjacent tile with additional discoloration, and other ceiling tiles that were cracked, broken, and sagging. The maintenance director confirmed the ceiling tile was stained and busted, and the administrator stated the stained and cracked ceiling tiles would be expected to be replaced.
