Repeated Omission of Hyponatremia Medication
Summary
The facility failed to ensure that a clinically significant medication for chronic hyponatremia was transcribed correctly, made available, and administered as prescribed for a resident with diagnoses including hypo-osmolality, hyponatremia, diabetes, chronic kidney disease, and SIADH. The resident’s records showed intact cognition on admission and a care plan that included medication administration per orders, a strict 2-liter fluid restriction, and laboratory monitoring. After hospital discharge, the resident’s [NAME]-Na order was entered into the facility MAR as 30 mg daily, which did not match the discharge summary directing 30 grams with meals, and there was no documentation that staff clarified the dosage or timing discrepancy with the provider or pharmacy. The resident’s MAR and progress notes documented repeated omissions of the medication. From 4/24/26 through 4/27/26, the resident had four omitted doses, with notes stating the medication was on order, but there was no documentation that nursing staff notified the provider or pharmacy about the missed doses. The resident later returned from the hospital with orders for [NAME]-Na 15 grams three times daily with meals, but the facility MAR showed the medication as unavailable or out of supply on multiple shifts from 5/5/26 through 5/12/26. The records identified that only 5 packets were delivered on 5/12/26, which was insufficient for the ordered dosing schedule, and the MAR and progress notes documented repeated omissions and shortages over the following days. The resident’s clinical condition declined during this period, with notes describing increasing weakness, inability to lift arms or legs, dependence for feeding and activities of daily living, shortness of breath, edema, crackles, and extreme thirst. The resident was transferred to the hospital multiple times, including after abnormal sodium levels were documented and later for worsening respiratory status and decline. The facility’s records showed a total of 28 omitted doses of the prescribed [NAME]-Na medication between 5/4/26 and 5/21/26, and there was no documentation that nursing staff notified the physician, PA, or NP about the repeated omissions or that an emergency procurement plan was used to obtain the medication from another source. During interview, the DON stated that the provider should have been notified when medication was unavailable and acknowledged that no medication error reports were completed for the omitted doses.
