Poor maintenance and cleaning of laundry and common areas

Medilodge Of Capital AreaLansing, Michigan Survey Completed on 06-26-2026

Summary

The facility failed to effectively clean and maintain the physical plant in the laundry area and several common areas. During an environmental tour of the laundry service, one of two commercial washing machines was observed non-functional, and a laundry aide stated it had been down for at least two months. The account manager stated the facility had purchased a new machine and was awaiting installation. In the clean laundry room, the hand sink aerator was partially obstructed with mineral deposits restricting water pressure, and four overhead protective light lens covers were heavily soiled with accumulated dust and dirt. In the soiled laundry room, one overhead light lens cover was heavily soiled, two vinyl flooring tiles were missing near the entrance door, and four laundry carts and two large plastic bins were completely full and overflowing with soiled laundry, with additional soiled laundry mounded along one wall approximately 12 inches from the ceiling. The account manager stated the soiled laundry was backed up because only one washer was available. Additional observations in other areas showed maintenance and cleaning issues. In the 100 Hall memory care unit dining room, the hand sink hot water supply faucet was leaking. In the spa, two of four overhead light assemblies and the over-sink light assembly were non-functional. In the 500 Hall activity room, the oscillating wall-mounted fan was soiled with accumulated and encrusted dust and dirt deposits. Record review showed the facility’s Preventative Maintenance Program required a safe, functional, sanitary, and comfortable environment, and the Routine Cleaning and Disinfection policy required routine cleaning and disinfection to provide a safe, sanitary environment and prevent infection transmission to the extent possible. Review of the Electronic Maintenance Software System work orders for the last 90 days revealed no specific entries related to the cited maintenance issues.
