Summary

The facility failed to ensure podiatry care was provided to one resident who was reviewed for foot care. The resident was admitted with diagnoses including Alzheimer's, moderate malnutrition, adjustment disorder, kidney failure, and diabetes, and was wheelchair/bed bound with assistance needed for positioning, toileting, and hygiene/grooming. On observation, the resident's feet had long, hard, discolored, dry, and brittle toenails extending beyond the toe bed, and flaking debris resembling dead skin was seen on the bed sheet beneath the feet. The social worker stated the resident had been referred several times, including in October 2025, February 2026, and June 2026, but the podiatrist never had the resident on the list during facility visits. The podiatry customer service representative stated the resident was on a do not treat list because required documentation, including a current request for service form, an up-to-date medical order, and a face sheet, had not been received.
