Summary

The facility failed to effectively clean and maintain the physical plant during environmental tours and record review, affecting 91 residents. On 07/21/2026, surveyors observed heavily and moderately soiled return air exhaust ventilation grills in soiled utility rooms on the 2 East Unit and 2 [NAME] Unit, and on the 3 [NAME] Unit they observed a missing and loose perimeter wall/floor coving strip in the janitor closet and a cracked and broken overhead light lens cover in the soiled utility room. On 07/21/2026, a resident room was observed in complete disarray with paper wrappers, food residue, a bedside commode containing aromatic urine and feces, soiled and stained bed linens, a commode that would not flush, a hand sink that drained slowly, and a non-functional PTAC unit, with a temporary portable air conditioning unit in use. On 07/22/2026, additional observations included a loose quarter round base shoe in the 1 East Unit dining room, a microwave oven with a pitted, stained, corroded, and etched interior surface in Occupational/Physical Therapy, a shower room on the 2 [NAME] Unit with 24 missing ceramic tiles and a soiled oxygen room floor, and on the 3 [NAME] Unit a loose commode base and support assembly, stained and warped ceiling tiles, soiled return-air exhaust ventilation grills, and another resident room in complete disarray with heavily soiled linens and a bedside commode partially full of feces and urine. Record review showed maintenance and housekeeping policies requiring proper maintenance and daily cleaning of non-carpeted floors and other horizontal surfaces, and the electronic maintenance monitoring system had no specific work order entries related to the cited concerns in the last 90 days.
