Summary

The facility failed to ensure a sit-to-stand lift was used correctly for one resident who had a history of right femur fracture, osteoarthritis, obesity, muscle weakness, and cognitive communication deficit. The resident was being assisted from the toilet in the bathroom when she slipped from the lift platform during the transfer. Review of the incident showed that the resident was partially off the standing platform, with one leg on the platform and the other leg on the floor, while staff lowered her to the floor. The resident’s fall investigation and witness statements identified that required safety devices on the sit-to-stand lift were not secured during the transfer. The CNA involved reported that the resident was leaning to the left and that the safety belt and shin/leg support buckle were not secured. Another staff member observed the resident in an awkward position with one knee bent down while both arms remained on the lift. Nursing documentation also described the resident’s right knee on the standing platform, the right foot on the floor, and the left leg still on the lift platform. After the incident, the resident had right knee pain, a superficial skin tear, and bruising to the right knee. Imaging at the facility showed a displaced fracture of the distal femur, and hospital imaging confirmed a displaced distal right femur fracture. The resident later underwent surgical fixation with insertion of an intramedullary rod. Interviews with the DON, CNA, RN, and therapy director confirmed that the resident had fallen during the transfer because the lower leg strap and torso strap were not secured as required.
