PASARR screenings and follow-up reviews not completed for residents with mental health and cognitive diagnoses

Froh Community HomeSturgis, Michigan Survey Completed on 06-11-2026

Summary

The facility failed to ensure annual PASARR Level I screenings and follow-up Level II screenings were completed for multiple residents with mental illness, intellectual disability, dementia, and related diagnoses. The report identified five residents reviewed for PASARR who did not have the required screening documentation completed or maintained in their records, and several care plans did not include a PASARR/OBRA/Level Screening focus for mental health needs. The deficiency was based on interview and record review and was found during the survey process. For one resident, the record showed diagnoses including severe dementia with agitation, psychotic disorder with delusions, depression, anxiety, and unspecified intellectual disabilities. The resident’s PASARR Level I screening indicated multiple “yes” responses for mental illness, treatment for mental illness, prescribed psychotropic medications, evidence of mental illness or dementia, and intellectual disability, which required referral for Level II review. However, the chart did not contain an annual Level I PASARR or a follow-up Level II PASARR after the prior screening. During interviews, the DON and SSD stated they would look for the documentation, and the DON later stated that after the former social worker left, the Level I PASARR screenings and the documentation being sent to CMH for Level II review were not being done and she was unsure how many residents were missed. Another resident had a diagnosis of bipolar disorder, a BIMS score indicating intact cognition, and use of antipsychotic and anticonvulsant medications. The resident’s Level I PASARR screening showed current mental illness and treatment for mental illness, but the record did not contain a 2026 annual Level I screening or a Level II screening completed by community mental health. The resident’s care plan also lacked a PASARR/OBRA/Level Screening focus. In interview, the SS stated he was not a licensed social worker and could not complete the Level I screening, while the DON stated she had thought the SS could complete the OBRA forms and that the community mental health agency would do the Level II screenings. Additional residents had similar gaps. One resident with non-Alzheimer’s dementia had a Level I PASARR indicating mental illness and dementia, but no 2026 annual Level I screening was found and the Level II screening record did not show it had been sent for community mental health review; the care plan lacked a PASARR/OBRA focus. Another resident with severe cognitive impairment and diagnoses including dementia, anxiety, depression, schizophrenia, schizoaffective disorder bipolar type, and borderline personality disorder had a Level II screening that required reevaluation within 363 days due to a change in condition, but no completed reevaluation was found. A fifth resident with dementia and schizophrenia had a Level I screening indicating mental illness and dementia, but the record did not show a completed Level II screening for the year and the care plan did not include a PASARR/OBRA/Level Screening focus for mental health needs.

F0645 F645: PASARR screening for Mental disorders or Intellectual Disabilities
Short Summary

PASARR screening was inaccurate for two residents. One resident had bipolar disorder and a Level I PASARR showing a need for Level II review, but the required Level II evaluation was not obtained before or after admission. Another resident was admitted with major depressive disorder, personality disorder, substance abuse, and PTSD, and the Level I PASARR also indicated a Level II evaluation was required prior to admission; the DON later confirmed the PASARR was incorrect and should have been caught on admission and during audit review.
