Summary

The facility failed to ensure a newly admitted resident with type 2 diabetes mellitus received ordered blood sugar monitoring and insulin therapy. The resident’s hospital discharge summary documented uncontrolled diabetes with an A1c of 10.4 and discharge orders for Humalog 10 units before meals, Toujeo 22 units at bedtime, and daily blood sugar checks. The baseline care plan did not include a diabetes plan. After admission, the resident’s insulin orders were changed in the electronic record when the pharmacy substituted equivalent medications, but the new orders remained in the pending confirmation status and were not activated in the MAR. Nursing staff did not confirm the pending orders at the start of shifts, and the resident’s blood sugar checks and insulin doses were missed for a period after admission. The resident’s family member became upset after learning the resident had not received insulin for two days and removed the resident from the facility.
