Failure to Follow Transfer and Toileting Care Plan

Summary

The facility failed to implement care planned interventions for transfers and toileting for a resident who had fractures of the right fibula and left calcaneus, multiple falls, obesity, and was completely dependent on staff for transfers and toileting. The resident’s care plan called for assistance of 2 people with a hoyer lift for transfers, extensive assistance from 2 people for bed mobility, and dependent assistance from 2 people for toileting, with a bed pan to be offered if the resident needed the bathroom urgently. The resident’s fall care plan also identified an increased fall risk related to a history of falls, fibula fracture, weakness, and anemia. On the morning of the incident, the resident was in bed screaming for help and saying she needed to use the toilet. Staff attempted to move her without using the hoyer lift because she would not allow staff to get the hoyer and because she was trying to get out of bed. A CNA assisted the resident to sit on the side of the bed, used a gait belt, lifted and pivoted her into a wheelchair, then transferred her again onto the toilet. The CNA reported that the resident did not have a bedside commode and that she was not able to wait for another staff member or a nurse because the resident was trying to get out of bed. After the resident finished on the toilet, two CNAs attempted to stand her up and transfer her back to the wheelchair. The resident said she was too tired, her legs gave out, and she was slowly lowered to the floor. Staff then rolled her side to side to place a hoyer sling underneath her and transferred her back to bed using the hoyer. The LPN later stated she was told about the incident after lunch, did not complete a range of motion assessment, did not document the incident, and did not report it to management because she believed it was not a fall if there were no injuries. The DON later confirmed the resident was only supposed to be transferred using the mechanical hoyer lift and that the staff should have called for assistance before attempting the transfer.
