Summary

The facility failed to implement fall prevention interventions for a resident with severe cognitive impairment, dementia, anxiety disorder, and a history of falls after identifying the root cause of an unwitnessed fall involving a power lift recliner. The resident was found on the floor with the recliner fully reclined after the remote control was activated, and the investigation identified unsafe chair positioning, impaired safety awareness, and access to the recliner remote as contributing factors. A resident with dementia, rheumatoid arthritis, muscle weakness, and moderate cognitive impairment was observed at breakfast with coffee in an uncovered cup and appeared to be falling asleep at the table before spilling coffee onto her lap. On later observations, the resident again had coffee without a lid even though the care plan directed staff to offer EZ lids for coffee, and the meal ticket specified coffee with an EZ lid. Staff stated they did not know to provide the lid and had not read the ticket tray, and the DON confirmed the resident should have had a lid for her hot coffee.
