Infection Control and Water Management Deficiencies

Brittany ManorMidland, Michigan Survey Completed on 05-13-2026

Summary

Provide and implement an infection prevention and control program was deficient because staff did not follow standard infection control practices during medication administration for one resident and the facility did not have an active and ongoing plan for reducing the risk of legionella and other opportunistic pathogens of premise plumbing. R24 was admitted to the facility on 8/27/2025 with diagnoses that included congestive heart failure and received daily medications from facility staff. The facility policy for medication administration required hand hygiene prior to medication preparation and directed staff to place medications into a cup without touching the inside of the cup, and to discard medications that came into contact with bare hands. During observation on 5/13/2026, RN L was seen preparing R24's oral medications by placing stock and prescription medications into his bare hands before putting them into a medication cup. Aspirin 81 mg was poured into the nurse's bare hand, one tablet was selected, the unneeded tablets were poured back into the stock container, and the selected tablet was placed into the cup by hand; the same method was used with stock magnesium oxide 400 mg and vitamin D3 capsules. Other medications, including pyridostigmine bromide 60 mg, Lyrica 100 mg, linaclotide 72 mcg, and Lexapro 5 mg, were also directly deposited into the nurse's bare hand before being placed in the medication cup and administered to R24. RN L stated this was usually what he did, and RN UM C stated the method was not consistent with facility policy or standards of practice. For the water management component, review of the water management plan found no documentation of a water management team meeting, risk assessment, or review of policy or assessment for 2025 or 2026. The maintenance director could not locate the requested meeting minutes, agenda, or completed risk assessment forms for 2025, stated the former maintenance director had not left the documentation, and the NHA indicated the water management team meeting and risk assessment had not been completed for the current year.

D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A resident being evaluated for C. difficile was not consistently managed under the correct contact-enteric precautions. Failure to Use EBP PPE During Wound Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Failure to Use EBP PPE During Wound Care: A resident with a wound and severely impaired cognition was receiving ordered wound care when the DON and ADON entered the room, washed hands, and donned gloves but did not wear gowns before starting care. EBP signage and PPE were present at the bedside, and both leaders later stated they forgot to put on gowns even though the resident was on EBP for an open wound and the facility policy required gown and glove use for wound care.

Failure to Follow Enhanced Barrier Precautions and Hand Hygiene During Resident Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Staff failed to consistently follow EBP and hand hygiene for two residents who required high-contact care. One resident with an indwelling catheter and cognitive impairment did not have EBP followed during transfers, clothing removal, and pericare, and another resident with a suprapubic catheter and multiple pressure ulcers had staff wear PPE incorrectly, remove PPE in the room without hand hygiene, and continue care after emptying the catheter bag without changing gloves or cleaning hands. Interviews confirmed staff were expected to use gowns, gloves, and hand hygiene for these tasks.

Failure to Use Required PPE for Resident on Contact Precautions
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Although signage outside the room directed all entrants to wear a gown and gloves, an RN entered the room with medications without either item and touched the bedside table and door surface. The resident stated staff did not always wear gowns, and the DON confirmed staff were expected to wear a gown and gloves when entering the room while precautions were in place.

E
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A facility failed to maintain infection control during care for two residents. During incontinence care, a CNA removed gloves and applied clean gloves without hand hygiene before placing a clean brief on a resident who was dependent for toileting hygiene and always incontinent. During ostomy care, an LVN handled a resident’s colostomy, wiped stool from the stoma, and continued care without removing soiled gloves, sanitizing hands, or putting on clean gloves. The DON stated staff were expected to perform hand hygiene between glove changes.

Infection Control Failures During Incontinent Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Infection Control Failures During Incontinent Care: Two residents were observed receiving incontinent care with multiple infection control lapses. A CNA did not perform hand hygiene between glove changes or after glove removal while caring for one resident, and another CNA did not wash hands before care, changed nothing between dirty and clean tasks, touched clean items with dirty gloves, placed soiled linens on the floor, and handled dirty linens with bare hands after glove removal. The DON stated hand hygiene, glove changes, and proper handling of soiled linens were required, and facility policies reflected those practices.
