Summary

The facility failed to maintain proper Enhanced Barrier Precautions for two residents with indwelling catheters, R14 and R41. During an observation, two CNAs entered R14’s room to complete a transfer using a lift; one CNA did not don gloves or a gown, and the other donned gloves but no gown. After the transfer, one CNA emptied R14’s Foley catheter and cleaned the catheter tip with an alcohol pad, then sanitized hands, and the other CNA returned to make the bed without donning a gown or gloves. Both CNAs stated they did not understand the purpose of the sign on the door and reported they had not received education on Enhanced Barrier Precautions. Review of active orders showed R14 had an order for Enhanced Barrier Precautions related to an indwelling catheter. A similar observation occurred in R41’s room when two CNAs entered to complete a lift transfer and both donned gloves but no gown. After the transfer, they exited the room after sanitizing their hands. Both CNAs stated the sign on the door meant to gown up only for catheter care, despite the resident having an active order for Enhanced Barrier Precautions related to an indwelling catheter. The facility also failed to fully implement its water management program when two capped water lines under the dirty dish table were observed; the Maintenance Director initially stated the lines were not on the flushing schedule and was unsure whether they had access to water, then later confirmed they did have access to water after removing a cap and turning on the line. The undated Water Management Program policy stated an annual review and risk assessment would take place.
