Unsafe and Uncomfortable Facility Environment

The Nursing And Rehab Center At Stadium PlaceBaltimore, Maryland Survey Completed on 07-14-2026

Summary

The facility failed to maintain a functional and comfortable environment on 4 out of 4 units reviewed for environment. On the 1st floor shared bathroom, the walls were bubbled with unpainted repairs on the opposing walls. When asked about the repairs, Staff #1 stated that repairs had already been completed on the 2nd and 3rd floors and that the 1st floor shared bathroom would be repaired next. Staff #1 also stated that administration had been made aware that paper towels were causing the pipes to clog, which resulted in leaks and damage to the walls of the lower bathrooms. The maintenance director stated that the paper towel dispensers had been replaced with an air drying system in the bathrooms. Resident interview and staff interview also identified temperature concerns in the facility. Resident #1 stated that the temperature was fine at the time of interview but that the weekend had been rough because it was too hot. Staff #1 stated that portable air units were in the main hallways because the A/C unit had been under repair since his arrival about a year earlier. Staff #1 stated that the A/C system was running at its highest capacity and only maintenance was required at that time, but also stated that the unit would have to be replaced or upgraded for the facility temperature to be maintained without portable units. The Administrator stated that the portable A/C units had been supporting the main units since their arrival in the beginning of June 2026.
