Summary

The facility failed to ensure that a resident was served a meal according to a predetermined menu that incorporated the resident’s preferences. The resident was receiving hospice services and had a current plan to transfer to another SNF where the spouse was residing. During lunch observation, the resident was being assisted in the dining room while on a mechanical soft diet with nectar thickened liquids, and the meal ticket indicated preferences for additional gravy and use of a translucent small sip cup with 2 handles. Review of the resident’s lunch tray showed that three items listed on the meal ticket were missing: a pureed cranberry crunch bar, one serving of a frozen nutritional treat (8 ounces of cranberry juice), and 1/2 cup of mandarin oranges. The GNA assisting the resident was not aware why the items were not sent with the tray. RN brought the missing items to the resident later, and the dietician confirmed the resident’s ordered dysphagia mechanical ground regular diet with nectar thickened liquids, along with documented meal choices, supplements, and adaptive equipment on the nutritional care plan.
