Food Storage, Sanitation, Temperature Monitoring, and Plumbing Deficiencies

Windward GardensCamden, Maine Survey Completed on 05-20-2026

Summary

Food was not stored, prepared, and served in accordance with professional standards for food service safety. In the walk-in refrigerator, surveyors and the Food Service Manager observed open beverage containers belonging to dietary staff stored with resident food, an open package of hot dogs labeled with an expiration date of 5/17/26, an open 5-pound package of shredded cheese labeled with an expiration date of 5/15/26, and a container of chopped lettuce labeled with an expiration date of 5/17/26. In dry storage, an open 5-pound package of seasoned breadcrumbs was observed with an open date of 4/8/26 and an expiration date of 5/8/26. Surveyors also observed food debris on shelves around stacked clean dishes, a rubber glove between two piles of clean plates, and kitchen surfaces that were moderately soiled with food debris unrelated to the meal service. During meal service, a cook used a wet cloth that had been laying on a countertop to wipe a food thermometer, and the cloth appeared to have green food debris on it. After the cloth was thrown into a trash bin, the cook closed the lid with bare hands and then touched the thermometer without washing hands. The cook later confirmed she had not taken temperatures on the mechanical soft turkey, pureed turkey, green bean and potato puree, or either pan of gravy. Surveyors and the FSM then confirmed holding temperatures of 123.6 degrees F for the mechanical soft turkey, 122.6 degrees F for the pureed turkey, and 118 degrees F for the green bean and potato puree, below the stated minimum holding temperature of 135 degrees F. Additional observations on the Spring Gardens Unit included an undated opened container of thickened apple juice, opened food items kept beyond the stated 3-day limit, and an improper air gap on the ice machine. Similar improper air gaps were also observed on ice machines on the North Wind Unit and Windward Center unit.

The dietary manager was unsure when the ice machine was last cleaned or how often the filter was changed, while the ADM believed it had been checked and cleaned within the prior week. The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
