Housekeeping and Maintenance Deficiencies in Resident Areas

Oak Grove CenterWaterville, Maine Survey Completed on 06-04-2026

Summary

The facility failed to adequately provide housekeeping and maintenance services necessary to maintain the building in a sanitary, orderly, and comfortable environment on 3 of 3 units and during 2 of 2 facility tours. On 6/1/26, a surveyor observed in a resident room shared bathroom a bedpan on the floor next to the toilet that was unbagged and unlabeled. Later that evening, a surveyor observed in another resident room bathroom a dirty, unlabeled bed pan on the back of the toilet with dried urine in it. In both instances, facility staff confirmed the findings during interview. During an environmental tour on 6/4/26 with the Senior Maintenance Director, Environmental Services District Manager, Administrator, and facility Maintenance Director, multiple additional concerns were observed and discussed. These included an unbagged commode lid on the bathroom floor, dirty floors around toilet bases, a dusty and dirty bathroom exhaust vent, a ceiling tile with a brown stain, chipped and missing paint exposing sheetrock on a resident room wall, an unlabeled urine hat, unlabeled urinal, unlabeled spray, and unlabeled cream on a bathroom toilet, marked and marred bathroom walls, chipped and missing paint on a bathroom door and door frame, used gloves on a shared sink counter, and an unlabeled container of Desenex powder, an unlabeled tube of Aquaphor cream, and an unlabeled bottle of Remedy body lotion on a wheeled plastic drawer unit. The Senior Maintenance Director, Environmental Services District Manager, Administrator, and facility Maintenance Director confirmed the observed findings.
