Failure to provide ordered symptom treatment, improper code-status discussion during emergency, and missing neuro checks after unwitnessed falls
Summary
The facility failed to follow provider orders for treatment of a resident with frequent loose and watery stools. Resident #69 had documented loose or watery stools on 19 of 24 days reviewed, including 13 of 15 days in June and 6 of 8 days in July, with a 6-day hospitalization during the review period. The resident had orders for Loperamide 2 mg every 6 hours as needed for loose stools and Pink Bismuth 262 mg/15 mL, 30 mL every 4 hours as needed for diarrhea, but the medication administration record showed the resident did not receive Loperamide in July and missed multiple ordered doses in June, and did not receive Pink Bismuth on the days loose or watery stools were documented in July and on most days in June. During interview, staff confirmed the resident was not provided the ordered PRN medications to treat the loose and watery stools. The facility also attempted to have Resident #69 sign a DNR while the resident was experiencing a medical emergency and was not reliably able to answer questions. The resident had a FULL CODE order and a POLST indicating Full Treatment, including CPR, hospital transfer if indicated, and intensive care. During the emergency, EMS documented that the resident was somnolent, more confused than normal, unable to reliably answer questions, and deemed unreliable; EMS also documented low blood pressures and that staff asked the resident whether he or she wanted to change DNR status. Facility statements and the APRN note described the resident as altered, diaphoretic, and not responding appropriately, while staff discussed DNR paperwork at the bedside as EMS was preparing transport. The facility also failed to complete neurological assessments after unwitnessed falls for Resident #10. The record showed multiple unwitnessed falls, but there was no evidence that neurological assessments were completed after those events. During interview, staff were unable to determine key details about the falls and stated that neurological assessments must be restarted with each unwitnessed fall. The facility’s clinical lead acknowledged that completion of neurological assessments had been identified as a problem, but the assessments were not completed after the unwitnessed falls reviewed.
