Summary

Drugs and biologicals were not consistently labeled and stored according to accepted medication storage practices. During observations of multiple medication preparation areas, a medication storage room, and a medication cart, surveyors found expired medications available for use, multi-dose medication containers without required opening dates, and an unlabeled medication cup containing unknown pills. The facility’s insulin policy required opened insulin to be dated and discarded after 28 days, and the medication policy required the opening date to be recorded on multi-dose containers. In one medication prep area, an opened and unrefrigerated Lispro insulin flex pen for one resident had been open for 32 days, and another resident’s opened and unrefrigerated Toujeo flex pen had been open for 40 days. In the medication storage room, a resident’s Tramadol 50 mg remained available even though it had an expiration date and 21 pills were still present. The RN who was present stated the insulin pens were expired and should not have been available for use, and that the expired Tramadol should have been removed from the storage room. In another medication prep area, an opened and unrefrigerated Novolog insulin flex pen for a resident did not have an opened date documented, and the LPN stated the expiration date was unknown and the pen should not have been available for use. On a medication cart, an opened and unrefrigerated Tymlos injection pen for another resident also lacked an opened date, despite directions on the pen to discard it after 30 days of opening. In a separate resident medication storage area, surveyors found a medication cup containing 17 unlabeled small white pills, and the RN confirmed she did not know what the pills were, when they were placed there, who placed them there, or when they expired.
