Chemical restraint given without required assessment or documentation
Summary
The facility failed to ensure a resident was free of chemical restraints when staff administered a B52 injection for aggressive behavior without documenting an assessment of the resident before the medication was given, without documenting attempts to identify possible underlying causes for the behavior, and without documenting attempts to use less restrictive interventions first. The resident had diagnoses including unspecified dementia with behavioral disturbance, intermittent explosive disorder, generalized anxiety disorder, major depressive disorder with psychotic symptoms, and pseudobulbar affect. The record showed the resident had episodes of verbal aggression and refusal of care, including an incident in which a nurse documented the resident smacking her hand away, cursing, refusing to return a BP cuff, and moving toward staff in a threatening manner. On 06/16/2026, orders were entered for diphenhydramine 50 mg IM, haloperidol 5 mg IM, and lorazepam 2 mg IM for aggressive behaviors toward staff, and the MAR showed all three medications were administered between 12:25 p.m. and 12:58 p.m. However, the progress notes did not document the specific behaviors present at the time of administration, any assessment of the resident's condition, or any attempt to identify underlying causes or use non-pharmacological interventions before the injection. The record also lacked documentation of any behavior monitoring for the resident during the month, including on the day the B52 was given, and there was no documentation that the resident was assessed after the injection to determine whether the symptoms resolved. During interviews, the DON confirmed there was no documentation of the resident's condition or behaviors before the injection and expected assessment before and after a chemical restraint. The PNP stated she would expect an assessment, including vital signs, after a heavy medication such as a B52 injection, and the LPN who administered the injection stated she did not document the resident's behaviors, contact the physician for orders, or document whether the medication was effective.
