Call Lights Not Kept Within Reach
Summary
The facility failed to ensure call lights were within reach for two residents who had care plans that included keeping the call light within reach. Resident #8 was admitted with acute on chronic diastolic congestive heart failure, had a BIMS score of 7 indicating severe cognitive impairment, and could make her needs known. Her care plan included assistance with ADLs, fall potential, and an intervention to keep the call light within reach. During multiple observations, her call light was clipped to the curtain pushed toward the back wall, clipped to the outer edge of the bed, and again clipped to the outer edge of the bed, all out of her reach. She stated she used the call light to request help, reported she could not find it, and said it was not usually within reach. A CNA later confirmed the call light was not within reach and stated it should have been placed within reach every time staff entered the room. Resident #76 was admitted with diagnoses including unspecified dementia, contractures, dysphagia, and hemiplegia and hemiparesis following a nontraumatic intracerebral hemorrhage affecting the left non-dominant side. Her quarterly MDS showed a BIMS score of 8, indicating moderate cognitive impairment. She was bed and geriatric chair bound, incontinent of bowel and bladder, required a mechanical lift for transfers, and her care plan included ensuring the call light was within reach and encouraging her to use it for assistance. On several observations, her call light was clipped to the side of the mattress, attached at head level, and hanging under the bed, none of which were within her reach. She attempted to reach it and stated she could not reach or find it. A CNA supervisor and the ADON both confirmed the call light was not within reach and stated all call lights should have been within residents' reach at all times.
