Dirty Shower Area and Resident Cup Observed

Jeff Davis Living Center, LlcJennings, Louisiana Survey Completed on 05-13-2026

Summary

The facility failed to ensure a safe, clean, and homelike environment for Resident #50 by not keeping the resident’s shower area and shower chair clean before use. Resident #50 had diagnoses including legal blindness, dermatitis, rheumatoid arthritis, unsteadiness on feet, cognitive communication deficit, and a history of falls. Her most recent MDS indicated a Brief Mental Status score of 12, showing her cognition was intact. During an observation of the resident’s bathroom and shower, the shower chair seat had dark patches of a black substance, the chair legs and rubber caps had a wet, velvety, film-like black substance, and the shower walls and floor had a black film-like substance with small black circular patches. Two wet washcloths were also observed on the shower floor, and the CNA confirmed they had been used to clean the resident and were left there dirty. The CNA stated she had assisted the resident with a shower that morning and had seen that the shower was dirty before providing care. She confirmed the shower should have been cleaned before the shower was given. The CNA also wiped the shower chair leg with a washcloth, and chunks of black sooty, sludgy-like substance came off onto the cloth. In addition, the resident’s clear plastic sippy cup with a blue lid and built-in straw, which she said she used every morning to rinse her mouth, was observed with a black substance inside the bottom of the cup and straw and small black circular patches on the inside walls near the bottom. The supervising CNA confirmed the cup was dirty and should have been cleaned. Housekeeping later confirmed that resident bathrooms and showers inside resident bathrooms were to be cleaned daily and deep cleaned weekly.
