Unnecessary Psychotropic Medication Use Without Documented Rationale
Summary
The facility failed to ensure that Resident 6 was free from antipsychotic medication use without an appropriate indication or a physician-documented rationale, including risks versus benefits and review of nonpharmacological interventions that had been attempted and failed. Resident 6’s record showed diagnoses including major depressive disorder, cerebral infarction, dementia with agitation, chronic pulmonary edema, Alzheimer’s disease with late onset, and dysphagia. The quarterly MDS documented intact cognition with inattention and disorganized thinking that fluctuated, no exhibited behaviors, substantial to maximal support needs, dependence for mobility, and receipt of an antipsychotic on a routine basis without a gradual dose reduction attempt or documented contraindication. The care plan initiated on 01/15/2026 identified psychotropic medication use, including Seroquel and Depakote, for behavior management related to dementia with agitation and major depressive disorder. It directed staff to administer medications as ordered, consult with pharmacy and the physician regarding dosage reduction when clinically appropriate, and monitor target behaviors, side effects, and adverse effects. The record included a progress note on 02/18/2026 documenting verbal aggression, cursing, yelling, and banging on the wall after staff attempted to assist with care. A physician order on 02/20/2026 directed Seroquel 25 mg at bedtime for a mood disorder and discontinued Abilify. Subsequent notes documented additional behaviors, including yelling out after meals, inappropriate sexual comments, and increased agitation with verbally abusive behavior. Orders later increased Seroquel to twice daily and then to morning and evening dosing. The EMR lacked documentation of nonpharmacological interventions attempted before Seroquel was started and lacked a physician-documented rationale that included risks versus benefits for its use. During interviews, staff reported that Resident 6 could be verbally and physically abusive at times, was often redirectable, and that behaviors and interventions were documented in progress notes, while an administrative nurse stated she was unaware of the indications for antipsychotic use.
