Food Storage, Dishwashing, and Equipment Sanitation Deficiencies

Medicalodges PaolaPaola, Kansas Survey Completed on 07-14-2026

Summary

The facility failed to store and prepare food under sanitary conditions. During observation of the kitchen and food storage areas, surveyors found a large storage bin of sugar in dry storage with no date, several small bowls of green pudding-like mixture in the walk-in refrigerator with no date and no label, and a lunch meat sandwich in the three-bay refrigerator with no label and no date. Dietary staff confirmed that food items should be labeled and dated. Surveyors also observed a white cutting board with several deep scratches on both sides and five silicone food scrapers with cracks and gouges on the sides, and dishes in the storage table were left face up and uncovered while a delivery person walked past them several times during food delivery. Surveyors observed the dishwasher in use and documented wash and rinse temperatures of 112 degrees F and 116 degrees F. The dishwasher temperature log for July 2026 showed no wash or rinse temperatures reaching 120 degrees F, with recorded wash temperatures ranging from 100 degrees F to 118 degrees F and rinse temperatures ranging from 106 degrees F to 118 degrees F. The dishwasher’s manufacturer plate stated a minimum of 120 degrees F for wash and rinse. Dietary staff reported documenting average temperatures rather than the actual readings and could not recall ever documenting 120 degrees F. The certified dietary manager stated she had been told the machine only needed to be 100 degrees F, but after reviewing the manufacturer label, she acknowledged the stated minimum temperature. Administrative staff also stated that food should be labeled and dated, the dishwasher should run at the correct temperature, dishes should be stored properly, and cooking equipment should be without scratches and cracks.

Short Summary

Improper Thawing of Frozen Chicken: Staff thawed frozen breaded chicken in a 3-compartment sink without cold running water instead of using an approved defrosting method. A Cook said she removed the chicken from the freezer about an hour earlier, and the DM confirmed the chicken was thawed in the sink after staff forgot to remove it from refrigeration earlier. The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.

D
F0812 F812: Procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards.
Short Summary

Ice Machine and Water Filter Not Properly Cleaned: An observation found black and brown substance on the ice machine's water curtain guard and a dark brown water filter behind the machine. The dietary manager was unsure when the ice machine was last cleaned or how often the filter was changed, while the ADM believed it had been checked and cleaned within the prior week. The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
