Summary

The facility failed to provide necessary ADL assistance for a resident with significant physical and cognitive impairments, including subarachnoid hemorrhage, lack of coordination, contractures, quadriplegia, neuropathy, and dementia. The resident’s MDS documented dependence on staff for bathing and all other ADLs, and care planning directed staff to provide substantial assistance with oral hygiene, toileting hygiene, and bathing, including sponge bathing with washcloths and soapy water if bathing was refused. An administrative nurse stated the resident was dependent on staff for mechanical-lift transfers and that shower caps should be used when bed baths were provided.
