Hand Hygiene Not Performed During Glove Changes in Wound Care

Prestige Care Center Of FairfieldFairfield, Iowa Survey Completed on 07-01-2026

Summary

The facility failed to ensure staff performed hand hygiene during glove changes while providing wound care to a resident with multiple sclerosis, heart failure, and peripheral vascular disease. The resident’s MDS dated 4/24/26 and an IDT note dated 6/25/26 documented a non-pressure skin issue to the right great toe, and the physician ordered Triple Antibiotic Ointment and an Optifoam Gentle 3 dressing to be applied daily. During observation of wound care on 6/30/26, the ARNP and the Infection Preventionist/LPN sanitized their hands and prepared supplies, then donned gloves to begin care on the resident’s right great toe, which had deep red bloody drainage and visible blood at the wound site. During the wound care, the ARNP removed gloves multiple times and failed to perform hand hygiene before donning new gloves. She removed the dressing, cleaned the wound, applied antibiotic ointment, measured the wound, applied foam dressing and Aquaphor to both lower legs, and at one point donned a glove on only one hand before touching her hair and opening and closing the resident’s nightstand drawer to obtain tubigrips. She also handled the resident’s dirty breakfast dishes with gloved hands before leaving the room. Staff interviews confirmed hand hygiene should occur before wound care and with every glove change, and the ARNP stated she knew she had forgotten to perform hand hygiene during glove changes. The DON stated staff should perform hand hygiene with every glove change, and the facility policy stated gloves do not replace hand hygiene and that hand hygiene should be performed before donning gloves and immediately after removing them.

F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Staff failed to consistently follow EBP and hand hygiene for two residents who required high-contact care. One resident with an indwelling catheter and cognitive impairment did not have EBP followed during transfers, clothing removal, and pericare, and another resident with a suprapubic catheter and multiple pressure ulcers had staff wear PPE incorrectly, remove PPE in the room without hand hygiene, and continue care after emptying the catheter bag without changing gloves or cleaning hands. Interviews confirmed staff were expected to use gowns, gloves, and hand hygiene for these tasks.
