Failure to Report Alleged Abuse

Prestige Care Center Of FairfieldFairfield, Iowa Survey Completed on 07-01-2026

Summary

The facility failed to report an allegation of abuse to the State Agency for 1 of 1 resident reviewed for abuse. Resident #40 had a BIMS score of 15 out of 15, indicating intact cognition. Staff interviews described an incident in which a Dietary Aide became upset while remaking the resident’s plate and used inappropriate and profane language toward the resident, including calling the resident a derogatory name and telling the resident, “You should know what you wanted.” Staff D stated he heard the exchange in early June 2026 but did not report it, believing Staff G would do so. Additional interviews showed the incident was known to multiple staff members but was not reported to the Administrator or State Agency. Staff F, RN, stated she was told the Dietary Aide was rude to the resident and later learned the aide admitted cussing in front of the resident, but she did not report it because she thought another staff member had. Staff G, CMA, stated the Dietary Aide told the resident, “you didn’t have to be such a dick,” continued using profanity, and left only after being told to leave; she also did not report it because she thought the nurses would. The Administrator stated she knew only that the Dietary Aide had gotten loud in the dining room and held a meeting with dietary staff, but said she would have suspended him and reported the incident if she had known what was specifically said. Review of the facility’s incident reports found no report to the State Agency regarding the Dietary Aide and Resident #40, despite the facility policy requiring reporting of alleged abuse within specified timeframes.
