Failure to Monitor QAPI and Resident Care
Summary
The facility failed to effectively monitor and improve the quality of care residents received under its QAPI/QAA process. The report states that, despite a written QAPI plan describing a data-driven, facility-wide program with monthly QAA committee review, the facility continued to have repeat deficiencies and multiple substandard quality of care tags. Survey outcomes showed 12 deficiencies on the prior recertification survey and 33 deficient practices on the current survey, including 6 substandard quality of care tags, with 3 rising to immediate jeopardy. The facility had been operating under a Directed Plan of Correction with monthly consultant reviews and biweekly calls. Those reviews documented trends in falls, wounds, medication errors, and smoking-related concerns, and the facility reported various monitoring activities such as audits, huddles, rounds, and tracking of incidents. However, the current survey found that leadership did not identify current issues with staffing levels, failure to provide cares such as repositioning, failure to keep pressure sore interventions in place, failure to perform pain assessments, inadequate incontinence care, and failure to maintain an environment free of urine odor. The monthly grievance logs from January through early May 2026 showed 75 grievances filed by 26 different residents, representatives, or staff. The Administrator stated that grievances were routed to different department leaders depending on the issue and that she would sometimes follow up with families and discuss concerns in huddles. The survey also identified repeat deficiencies in areas including resident rights, care planning, significant medication errors, resident records, sufficient nursing staff, and infection prevention and control, along with new immediate jeopardy findings for lack of comprehensive nursing assessment and intervention for 7 residents, lack of pressure sore interventions and care for 4 residents, and inadequate pain management for 1 resident.
