Unsanitary food storage, preparation, and service practices
Summary
The facility failed to store, prepare, distribute, and serve food under sanitary conditions. During kitchen observations, dirt and food were seen on the floor throughout the kitchen, along with old debris under counters and behind and around appliances, including dead cockroaches, discarded resident plates, plastic cups, paper plates, and forks covered in dust and grease. A sanitizer water bucket used to clean and sanitize kitchen surfaces tested at 50 ppm, below the manufacturer-required range of 100 ppm to 400 ppm, and the Assistant Dietary Manager stated the sanitizer water had not been changed since the start of the shift. Refrigeration and freezer practices were also deficient. Temperature logs for all refrigeration and freezer units had the afternoon dates for the same day already completed before the temperatures were actually recorded. In the main walk-in freezer, frozen food boxes were stacked directly against cooling fans and up to the ceiling, blocking airflow. In the main walk-in refrigerator, boxes were stacked against the automated fire sprinkler line and ceiling pipes. Multiple expired or improperly dated food items were found, including poppyseed dressing, tartar sauce, yogurt, tossed salad with hard-boiled eggs, and opened nectar thickening liquid with no date opened or expiration date. The Assistant Dietary Manager stated the expired items should have been removed. Food service and dishwashing practices were also observed to be unsanitary. A large pool of standing water covered the dish room floor, with loose cardboard and rags under the dishwasher and standing water under the ice machine; the Assistant Dietary Manager said the pooling water had been an ongoing issue for months. During meal service, a Dietary Aide repeatedly placed her bare thumb on the eating surface of plates while plating entrees and touched her clothes and face without re-washing her hands after retrieving items from a refrigerator. Another employee was later observed serving plates with bare thumbs on the food-contact borders and carrying an uncovered bowl of ice cream with his thumb inside the bowl in contact with the ice cream.
