Wound treatments were not consistently implemented or documented
Summary
The facility failed to ensure wound treatments were implemented and consistently completed for two residents reviewed for wound care. For one resident with diabetes, peripheral vascular disease, osteomyelitis, and a recent left first toe and first metatarsal excision, hospital discharge instructions included antibiotics, supplements, nitroglycerin paste to the amputation site, and keeping the wound VAC dressings clean, dry, and intact until follow-up. After admission, the resident had a surgical incision to the left toes and later developed increased drainage and odor at the surgical site. Wound clinic evaluations then added new wound care orders, including calcium alginate with silver, gauze dressings, and betadine to the remaining toe wound, but the July TAR lacked the new and continued wound treatment orders. The resident stated that treatment to the left foot had not been completed for three days, and staff observations showed the dressing did not appear to have been completed when checked. Wound Nurse 6 stated the wound doctor had already been in and the treatment had been completed, but another nurse observed that it did not look completed. The wound nurse later stated she had already changed the dressing, and the tape on the bandage was dated later that day. The record also showed the resident’s second toe became dark in color, and the resident was later sent for emergency surgery for amputation of the left foot second toe. For the second resident, who had paraplegia and stage 4 pressure ulcers to the right buttock/ischium and lower spine, the June and July TARs lacked documentation for multiple wound treatments on several dates. The wound orders included cleansing, calcium alginate, super absorbent dressings, and wound VAC therapy with black foam and continuous suction. During interviews, Wound Nurse 6 stated she normally completed the wound treatments and that the nurses documented them after she did so. Other staff stated they documented the treatments when Wound Nurse 6 told them they were completed. The DON stated Wound Nurse 6 should document in the clinical record when she completed the treatments, and the facility policy stated wound treatments were to be documented on the TAR or in the electronic health record.
