Summary

The facility failed to ensure a safe, sanitary, and homelike environment for residents on multiple units. On the Memory Springs unit, surveyors observed urine odor between rooms 122-125, marred and dirty baseboards, dirt buildup on window sills and fixtures, holes and chipped or missing trim, stained furniture, a floor coming up by the double sink, and multiple resident room and bathroom surfaces that were marred, dusty, sticky, or stained. Several bathrooms had stained toilets, dirty or missing light fixture covers, drywall coming off, loose or missing baseboards, and dirt buildup around walls and trim. On East Hall and [NAME] Hall, surveyors observed additional environmental issues including drywall coming off walls, broken or missing chair rail, chipped closet trim, peeling paint, dusty light fixtures, stained toilets and sinks, dirt buildup around baseboards, and sticky floors. In the service hallway, trim around doorways was marred and baseboards had dirt buildup. In one room on East Hall, Resident D stated the missing or broken chair rail had been that way for months. The report also noted a linen closet with a soiled towel on the floor, gloves on the floor, missing cove base, loose drywall around a sprinkler, and debris on the floor. Facility staff acknowledged ongoing environmental and housekeeping concerns. The Administrator and Regional Consultant stated there was a bid for repairs to five rooms on the Memory Springs unit, but no start date was given. The Maintenance Director said housekeeping handled cleaning and that he relied on work orders and staff reports for needed repairs. The Housekeeping Director stated rooms had not all been getting cleaned daily due to staffing shortage, multiple cleaners had been tried on toilet stains, and deep cleaning was scheduled when residents moved out. The facility policy stated the environment should be safe, sanitary, functional, comfortable, and homelike, and that room contents and floors should be clean and in good repair.
