Summary

The facility failed to ensure a resident with dementia received appropriate supervision, monitoring, interventions, and protections when another resident repeatedly attempted to enter her room. Resident R, who had diagnoses including traumatic brain injury and dementia, returned to the facility after a medical procedure that required her head to be shaved. She reported that Resident Q repeatedly tried to enter her room after her return from the hospital, disturbed her several times a day, and took items from her room. Resident R stated she complained to aides, nurses, and her family, but the behavior continued. She also described feeling like she had no privacy, being anxious about falling asleep, and being scared about what could happen if Resident Q became upset. Staff interviews and record review showed that Resident Q, who had diagnoses including Alzheimer’s dementia and anxiety, continued to wander into other residents’ rooms and fixate on Resident R’s room and Resident S’s room. A QMA stated Resident Q continued to focus on Resident R and sometimes entered Resident S’s room as well. Shift report logs dated 6/17/26, 6/23/26, and 7/6/26 documented that Resident Q targeted Resident R’s room and Resident S’s room and attempted to exit the unit, but the record lacked documentation that a staff member had been assigned to monitor or shadow Resident Q. During continuous observation, Resident Q repeatedly wandered the hallway, approached the front unit door, and moved close to Resident R’s room. Multiple staff members attempted to redirect her with food items and activities, but she resisted, slapped away items, and refused engagement. After staff left the unit, Resident Q again approached Resident R’s closed door multiple times and walked away only when observed. Staff stated they could not keep Resident Q away from Resident R’s room, that she rummaged through Resident R’s belongings and took items, and that she attempted to enter the room daily. Resident Q’s care plan lacked implementation or revision to address her intrusive wandering and targeting of Residents R and S, and Resident R’s care plan lacked revision for her preference to keep others out of her room.
